This page will contain a simple online form to allow a candidate to fill in the required details and then submit the form.
To protect the form from spam a simple security code/captcha system will be put in place.
Submitted forms will be sent directly to a Raisly email address but in addition stored within a forms database within the admin area of the website so they can be viewed/retrieved at any point.
Fields for this form are:
• Candidate Full Name – Required field
• Home Telephone – Required field (they must provide at least one phone number whether it be a landline or mobile it won’t matter.
• Mobile telephone
• Email Address – Required field

RAISLY: Please check above and confirm what additional fields are needed for the form and which should be required fields i.e. the user cannot submit the form without entering the information.
Client Notes:
There will also need to be an option for them to upload their CV to accompany this form. Whilst this won’t be a required field we need to have this as an option – we will accept Word/PDF docs. These CVs should be emailed to us along with the submission form. – Not a problem
